New Patient Registration

Patient Registration

We are accepting New Patients if they live in our catchment area. In order to register fully with the Practice, all patients must first read our New Registration Document, and provided us with a:

Please come to the surgery and provide us with an address proof of identification (e.g. Utility Bill or Council Tax Bill), which shows your full name and current address which is dated within the last 3 months. In addition, we need a photo ID (e.g. Passport or Driving License). Please bring in the originals as we will check this and return them back to you. Each adult that intends to register will need to come in the surgery in order for us to verify their photo Identification. Each child over 2 years old will be invited for a quick health check with our Health Care Assistant. Please bring in your immunisation details in order for HCA to update your records. If you are suffering from chronic disease such as diabetes, COPD etc. then you may be invited for a blood test with the HCA. Each adult that intends to register needs to come in the surgery preferably in the afternoon after 2pm with their completed forms and original proofs. Please allow up to 2 weeks for registration to be completed.

Patient Access

We now offer our patients the use of a secure website on the Internet to:

  • View and request repeat medication
  • View, book, and cancel appointments
  • View information from your care record.

Please come to the surgery and register for this service. You will need to bring in photo ID and a proof of your current address.
